Welcome to Louisiana Healthcare Connections. We’re so glad to have you as our member and look forward to caring for you and your health.

Your new health insurance plan offers comprehensive medical care and benefits. It also includes valuable programs, educational tools and support.

You will receive a New Member Packet in the mail. It looks like this:

Please watch for this packet. It has important information about your plan and your Member ID card. You will need your card to make appointments, get care, and use your plan benefits.

Your next steps with your new plan:

Create an account on our secure online Member Portal.

It’s free and easy. Our portal is designed for your smartphone—so no need for a separate app. To register, click on “Create New Account” and follow the steps.

With your member account, you can:

Request a new member ID card.
Update your personal information.
Send us a message.
Check your healthy rewards balance.

Fill out your Health Needs Assessment.

It’s a survey where you can share information with us about your health. Your answers are important! They help us learn more about you and how we can better support your health goals.

To complete it online, log in to your member account. You can also complete it by calling 1-866-595-8133 (TTY 711). When you complete it within 30 days of the effective date on your Member ID card, you earn $25 in healthy rewards!

Confirm your PCP.

When you log into your member account, be sure you have a doctor listed on the home screen. This is your PCP. They are the main doctor you will see for yearly wellness check-ups, screenings, vaccinations, and minor illnesses and injuries. You have the freedom to change your PCP at any time.

If the provider listed is the one you want as your PCP, no need to do anything else! If not, you can choose or change your PCP. Login to your member account and click “Change my Primary Care Provider.” You’ll be taken to our online Find a Provider directory. To select a new provider, click the “Make My Primary Care Provider” link under their name.

If you do not have one listed, choose your PCP on the portal, or call us at 1-866-595-8133 (TTY 711). You can also use our Find a Provider tool to search for a provider who is located in your area.

If you’re pregnant, complete a Notification of Pregnancy form.

This is available in your Member Portal. When you complete it, you earn $30 in healthy rewards!

Download a digital ID card.

You can easily keep your Louisiana Healthcare Connections Member ID on your phone with the LA Wallet app.

You can also view, print out, and request a new Member ID card using your member account.
