Care management is a service we provide at no cost to our members who have complex or special healthcare needs. Our care managers are here to help our members build healthy lives. Here are the three things we want you to know about our care management program:

Our care managers help you use your benefits in the best possible way.

The healthcare system can be very complex. You may not know what benefits are available to help you manage your health. Our care managers can help you understand the system and how to use your benefits for the best possible results.

Our care managers help coordinate your care.

You may have more than one doctor or healthcare provider. Perhaps you have a Primary Care Physician, or PCP, and a specialist for a condition like diabetes, or a behavioral health provider. Our care managers can help you make sure all your doctors are working together to give you the best care possible.

Our care managers help you find the programs and tools you need for better health.

Being healthy takes more than just quality health insurance. Sometimes, you may need help with problems like shelter, food or transportation. Our care managers have many resources in place to help you find the services and programs that will meet your specific needs.

You can also talk to your doctor or other healthcare providers and ask them to refer you to our care management program.
