There is an increased incidence of illness and death among people with substance abuse or mental health conditions, often due to a lack of contact with primary care services. These conditions may make it more difficult for a patient to take care of physical health issues, which may go untreated and often could have been avoided through integrated care delivery.
What is integrated care?
Integrated care is an essential component in achieving the highest possible levels of wellness, quality of life, and health outcomes for your patients. At its core, it’s the systematic coordination of physical and behavioral healthcare. This comprehensive approach is based on effective communication and coordination of evidence-based primary care and behavioral health services.

How do I know if my practice is integrated?
The SAMHSA-HRSA Center for Integrated Health Solutions has developed an Integrated Practice Assessment Tool (IPAT) to help identify a practice’s level of integration. The IPAT is a user-friendly decision tree that asks eight simple, yes-or-no questions. It takes only four responses to determine your practice’s level of integration. You can take the IPAT for free here by entering your National Provider Identification (NPI) number.
