How to Refer Patients to Care Management - And Why

Our Care Management team plays a unique role in supporting your efforts to provide quality, coordinated and integrated care to your patients – our members. Composed of licensed behavioral health professionals, our care managers are here to assist members who have complex or chronic conditions that require coordination of services and periodic monitoring to achieve desirable outcomes.
Among their functions are:
- Early identification of members with special health care needs
- Assessment of members’ risk factors and needs
- Contact with high-risk members discharging from hospitals to ensure appropriate discharge appointments are arranged and members understand discharge instructions
- Active coordination of care in linking members to needed health practitioners and services
- Development of a care management plan of care
- Referrals and assistance to community resources and/or behavioral health practitioners
- Helping members overcome barriers to obtaining needed services or treatment compliance
Reasons you might refer a patient to Care Management:
- Non-compliance with treatment
- Consistently missed appointments for treatment or follow-up care
- Need for integrated behavioral health or personal care services
- High ED utilization rate
- Complex or chronic conditions that require integrated, coordinated care
How to refer a member to Care Management:
If you have a patient who is a Louisiana Healthcare Connections member, and you believe that patient would benefit from Care Management services, you can refer the patient by completing this form.
