MediTrans to Provide Medicaid NEMT and NEAT Effective May 1, 2025

Effective May 1, 2025, Louisiana Healthcare Connections will contract with MediTrans to coordinate non-emergency medical transportation (NEMT) and non-emergency ambulance transportation (NEAT) for our eligible Medicaid* members. This partnership brings new opportunities for reliable service and improved experience for members, providers, and drivers.

We are committed to making the transition as smooth as possible for our members and providers. While some contact information is changing, our members’ transportation benefits and your process for requesting patient transportation will not. If you have any questions or need help, please call our Provider Services team at 1-866-595-8133.

 * This change does not impact Medicare patients covered by Wellcare. Wellcare will continue to contract with MTM for Medicare NEMT.

To Schedule NEMT Services for Louisiana Healthcare Connections Members

For trips needed after April 30, MTM will transfer you to MediTrans for booking.

There’s no wrong door! Providers dialing the MediTrans member line (1-855-369-3723) will be transferred to their healthcare facility line.

Then, call MTM at 1-855-369-3723 to complete scheduling.
 

Fax your CAT form to: 337- 366-6760.

Then, call MediTrans’ dedicated healthcare facility line at 1-844-349-4324 to complete scheduling.

The nursing facility or other provider should call to request authorization and schedule non-emergency ambulance transportation at least 48 hours ahead of time. The Certification of Ambulance Transportation (CAT) Form (PDF) is required prior to scheduling. This form can also be found on our provider Manuals, Forms and Resources webpage under Physical Health Authorizations and Reporting.
